      • Saturday – by appointment only 
    • Physical Therapy Department Hours
      • Monday – Thursday: 7:30 am – 6:30 p.m.
      • Friday – 7:30 a.m. – 5:00 p.m.
      • Saturday – 8:00 a.m. – 12:00 p.m.

      • Please call us at (713) 984-1400 to schedule an appointment. When you are injured, we will see you the day that you call!

    • What to bring to your appointment
      • Your insurance card or other health coverage information and photo id.
      • You may be responsible for a copay or deductible. So, be prepared to make payment at the time service to prevent any delay of service.
      • Your medical records, clinic notes, or operative reports from your referring physician that relate to your condition
      • X-rays or MRI’s taken within the last 12 – 18 months, if available
      • Any braces or orthotics you have been using
      • If you are having your knee examined, please wear or bring shorts.

    • Worker’s Compensation
      • For patients whose injury is work related, you must provide AOKC with an authorization letter from your employer or worker’s compensation carrier. Please include a claim or case number, contact person and their phone number, and appropriate billing address.

        • (2) scan and email to info@aokc.net
        • or
        • (3) bring in to the office at the time of your appointment.
    • Billing Information
      • Please contact our office if you have any questions regarding your coverage or account balance. You will be responsible for any charges that are not covered under your insurance plan. Like most physicians’ offices, we request payment in full at the time of service. Exceptions include worker’s compensation, Medicare, and preferred provider contracts (co-pay only).

    • Emergency Information
      • In case of immediate emergency, call 911. Our center always has a physician on call in case of after-hours emergencies. If you need to contact a physician when the center is closed, please call the regular office number and our answering service will notify the physician on call to contact you.
