Minimally Invasive Lumbar Decompression (MILD)

Minimally invasive lumbar decompression (commonly known as the MILD procedure) is a surgical procedure designed to relieve pressure on the spinal nerves caused by lumbar spinal stenosis (LSS), which is the narrowing of the spinal canal in the lower back. Lumbar spinal stenosis can cause pain, numbness, and difficulty walking due to this narrowing of the spinal canal.

Procedure

Common conditions treated with Minimally Invasive Lumbar Decompression (MILD)

MILD is ideal for patients with a confirmed diagnosis of LSS who are experiencing significant pain and reduced function, and who have not achieved adequate relief from conservative treatments.

Patients with the following criteria may be good candidates:

  • Diagnosed with LSS via MRI or other imaging
  • Experiencing neurogenic claudication (pain, tingling, or numbness in the legs)
  • Unable to tolerate more invasive surgery due to age or medical conditions

Procedure

A thorough evaluation should be performed before the procedure to assess the severity of spinal stenosis which includes review of medical history, physical exam, and use diagnostic imaging X-rays and/or MRI.

The MILD procedure aims to relieve symptoms of spinal stenosis such as numbness, pain and difficulty walking by removing excess ligament tissue that compresses the spinal nerves.  This technique involves smaller incisions and less tissue damage compared to traditional surgery, allowing for quicker recovery and less postoperative pain.  It is performed as an outpatient procedure through a 5 mm incision, typically taking less than an hour, and often allows patients to resume normal activities within 24 hours.

Benefits of MILD

The MILD procedure offers several benefits for patients with lumbar spinal stenosis, including:

  • Minimally invasive: No stitches, minimal tissue damage
  • Outpatient treatment: Performed in a surgical center, not a hospital
  • Quick recovery: Return to regular activities in a few days
  • No general anesthesia: Performed under local anesthesia and mild sedation
  • Clinically proven results: Long-lasting pain relief and improved mobility
